NorthShore University HealthSystem is gathering information as part of developing a plan to improve health and quality of life in the community it serves.  We are reaching out to community members to gather input through a community survey so we may identify and respond to unmet health needs in the community. 

 

The survey consists of 36 questions in two parts and should take approximately 6-8 minutes to complete.  Questions in Part I are directed to your Individual health status.  Questions in Part II are directed to the Community's health status.

Key themes that emerge from the survey will be compiled by Crowe, LLP, an independent advisory firm assisting NorthShore University HealthSystem with our assessment.  Your responses are anonymous and will remain confidential.
